  • Have you experienced intense,phobic fears of certain places, situations or objects, which you sense may be out of proportion to the true danger? 

  • Do you frequently avoid social situations due to fear of judgment or scrutiny? Do you often worry about being judged socially?

  • Do you feel compelled to take certain actions to relieve anxiety? 

  • Have you experienced ongoing gastrointestinal (GI) problems, heart palpitations, frequent headaches, muscle tension, rapid breathing or other physical symptoms related to anxiety?
